About Our Programs
The Cardiovascular Health, Assessment, Management, and Prevention Service, or CHAMPS, offers several cardiovascular health programs. The outpatient program is designed for individuals who have experienced serious heart problems such as heart attack, angina pectoris, peripheral vascular disease, heart surgery (bypass or valve), balloon, stent, atherectomy, or laser treatment.
For individuals who are considered to be at high-risk of developing heart disease, CHAMPS also offers a prevention program designed to help modify cardiovascular disease risk factors such as lack of exercise, stress, high blood pressure, diabetes, obesity, and smoking.
