Examples of Insurance Coverage for Phase II Outpatient Program

Medicare
Patient eligibility:

  • documented acute MI in past 12 months
  • coronary artery bypass graft (CABG)
  • stable angina pectoris
  • PTCA
  • valve surgery
  • heart transplant

Duration of program:

  • 36 sessions
  • usually 3x/week for a 12 week period

Blue Cross/Blue Shield
Patient eligibility:

  • acute MI or recurrent infarctions
  • coronary artery bypass graft
  • valvular surgical procedures
  • coronary angioplasty (PTCA)
  • congestive heart failure (CHF)
  • cardiomyopathy
  • heart transplant (HTP)

Duration of program:

  • 18 sessions
  • must begin 8 weeks following hospital discharge

IowaCare

The Indigent Patient Care Program, also known as the State Papers Program, ended on June 30, 2005.

In its place, the Iowa Department of Human Services has established the IowaCare program.

Persons eligible for the IowaCare Program are those between the ages of 19-64 with incomes at or below 200% of the federal poverty level who do not have other forms of insurance. Application forms are available within UI Hospitals and Clinics at Social Services, as well as from the Department of Human Services.
