Coordinated Care For Complex Health Issues

University of Iowa Children's Hospital collaborated with Child Health Specialty Clinics, Iowa's Title V Program for Children with Special Health Care Needs, to develop Continuity of Care for children with complex health issues. The staff helps negotiate, coordinate, and obtain services for patients and families both within the hospital and in their home communities.

The staff provides physical and emotional support to families through service coordination and builds relationships and networks to improve continuity of care for these young patients.
