Visitor and Transfer Information


We at the University of Iowa Hospitals and Clinics (UIHC) Dialysis Center encourage our patients to travel and welcome you to our dialysis facility. The following guidelines have been established to ensure that your visit with us is enjoyable and safe.

The UIHC Dialysis Center is an eleven-station facility. Gambro machines, bicarbonate dialysis and hollow fiber dialyzers are used to provide the dialysis treatments.

We require that your medical information be sent to our facility as soon as possible. We cannot confirm your dialysis schedule without the information requested on the visitor/transfer form. Your medical records must include both Hepatitis B & C results drawn within the past 90 days. This policy is for your protection as well as that of the other patients and staff in our facility.

Below is our visitor/transfer form(s)/packet– your dialysis facility will complete and return it with the requested information. This information is required prior to giving final approval of your requested treatments.

You will be required to stop at our Registration area upon arrival to our dialysis facility at the UIHC so that we can bill Medicare/other insurance for the dialysis services provided to you. You must supply a copy of your Medicare/insurance cards at the Registration area.

If you decide to cancel your dialysis treatment at our facility we appreciate notification 72 hours in advance of your scheduled treatment. We understand the effort required to make travel plans. The information we have requested is important for us to provide a safe and pleasant stay with us. We look forward to meeting you and providing your dialysis treatment.


THE VISITOR AND TRANSFER PROCESS

Step 1: Call the unit at least 2 weeks in advance to determine if we can accommodate the requested dates of travel. For emergencies—call immediately.
