Blood-borne pathogen exposure (BBP)


UI Employees

  1. Must complete an Exposure Incident Form from Environmental Health and Safety website.
    • Confidential medical evaluation is performed face-to-face, which includes: evaluation of exposure, counseling, post-exposure medication, baseline and post-exposure lab testing, immunizations/immunoglobulin, scheduled follow-up appointments, informing person of lab results, and appropriate paperwork documentation.
    • Source identification and blood testing is performed if possible and risk factors/lab results shared with exposed employee.

  3. First Report of Injury
    • The employee is required to complete the First Report of Injury form via the Employee Self Service website. Login using your HawkID and password. Under the heading Benefits, select Workers Compensation - First Report of Injury and follow the instructions. Staff benefits office will record the information on the OSHA 300 Log.
