Appointment Scheduling
To schedule an appointment, call the UI Health Access toll free at 1-800-777-8442. If you know the specific clinic in which you need care, you may call the following numbers between 8 a.m. and 5 p.m.:
Oral and Maxillofacial Surgery — 319-356-2205
Prosthodontics — 319-356-2601
Please have the following information available when scheduling an appointment with Hospital Dentistry Insitute:
- Full name with correct spelling
- Date of birth
- Social Security number
- Mailing address
- Daytime phone number
- Chief complaint or reason for appointment
- Names of caregivers (if any) to whom a report should be sent
- Insurance information
Clinic Hours
Regular clinic hours are from 8 a.m. to 5 p.m. Length of the first visit varies depending on the nature of the appointment and the level of evaluation necessary. The appointment scheduler will let you know how long to plan for your visit.
Emergency and After Hours Care
If you need to contact the clinic after hours, please telephone 319-356-1616 and ask for the General Dentist Resident on call. If your circumstance is an emergency, you will be directed to the Emergency Treatment Center at UI Health Care.
Cancellations
Please contact our clinics 48 hours in advance in the event that you cannot keep your appointment.
