Refer a Patient

We welcome your patient referrals.

Services are aimed at women age 18 and older suffering from mood and/or anxiety disorders during pre-pregnancy planning, pregnancy, and the postpartum period (up to 12 months after delivery).

Each patient will be contacted to schedule an initial evaluation appointment.

Providers should use the Women's Wellness and Counseling Service referral form. Please Download the PDF form, print it out, and fax it to 319-356-7533.
