Patient Inclusion Requirement


  • You (the female) must have no IVF insurance coverage or designate to have no charges submitted to insurance. All fees/charges for the cycle accrue to the female. No charges for the treatment cycles will be billed to insurance for the male partner as these are included in the cost of the risk sharing cycle.
  • You must be less than 40 years of age at retrieval.
  • You must be a non smoker.
    • Smoking during treatment reduces pregnancy rates by 50% and voids the risk sharing agreement. We will assist you in smoking cessation before acceptance into the program; however, you can be tested during a treatment cycle. If positive for metabolites of tobacco, your participation in the risk sharing program is voided and you are responsible for fee for services.
  • You must have two ovaries and normal ovarian volumes (>3.5 cc). Antral follicle counts must be >12 on a screening ultrasound (day 1, 2, 3 or 4) at UIHC or > 20 antral follicles on your ultrasound at your new IVF appointment.
  • You must have a normal clomiphene citrate challenge test (clomiphene citrate to be taken cycle days 5-9) and normal FSH testing (day 2, 3 or 4 and day 10 FSH levels < 10) within the last 6 months at UIHC. Paired FSH levels < 10 from an outside lab utilizing the same reference standard for FSH (2nd IRP, WHO 78/549) will be accepted with written confirmation of standard.
  • You must not have any significant myomas, polyps, scarring or septum of the uterine cavity.
  • You must not have hydrosalpinges (fluid filled fallopian tubes).
  • You must not have more than 2 prior fresh IVF attempts (anywhere) and/or prior poor response to stimulation (fewer than 5 normal embryos to culture).
  • You must have sperm available for insemination of eggs.
    • This can include frozen sperm from husband (or donor) or frozen retrieved sperm. This DOES NOT include frozen testicular tissue in cases of non obstructed azoospermia.
    • If the sperm numbers are very low, you may be required to be evaluated by our male infertility specialist.
    • If the sperm count is <5 million motile sperm/mL, you must have a normal karyotype and Y chromosome microdeletion testing.
  • You must agree to fertilization of all mature oocytes (eggs) and freezing (cryopreservation) of extra embryos.
  • You must agree to culturing more embryos than transferred to allow for selection of the best possible embryos.
    • This provides the best chance of success while limiting the number of embryos transferred. Limiting the number of embryos transferred reduces the risk of high order multiple pregnancies.
