Special Conditions:
Individually arranged elective form must be submitted 4 weeks prior to beginning of rotation. It is most important that the Orthopaedic Surgery Education Office (Linda Croy) knows the location of the elective in order to obtain credit for the elective.

Evaluations:
Students are responsible for evaluations. You must pick them up from Linda Croy before the start of the elective.
