Pediatric Blood and Marrow Transplantation

Consultation


A consultation may be requested either by a referring physician or directly by a patient or patient's family. Please contact our program coordinator at (319) 356-4324 to schedule a consultation with one of our physicians. The physician seen will depend upon the disease, type of transplant potentially needed, and availability of the physician. Dr. Frederick Goldman, director of the program, will initially review the request and assign the appropriate physician. The director may also approve telephone consultations as many of our families are from out of state and the travel in quite an inconvenience.

The physician will need the following information to review for the consultation.  It is important that this information is provided prior to the appointment so that the physician may review it and request any additional information necessary. 

  • Summary history : A letter from the referring physician or clinic detailing the current status of the patient, history of the disease, any additional relevant information.  This letter should be as complete and thorough as possible and should also include a contact person from the referring office in the event that any questions arise.
  • Diagnosis or relapse slide : If the disease being treated is a malignancy we prefer to review the actual biopsy slides ourselves.
  • Specific treatment information : (for malignancies)
    • Total Anthracyclines (m2>) (a type of chemotheraputic drug).
    • Radiation history including details of ports.
  • Tissue typing information on patient and family (if available)
