To Schedule an Appointment

Undergoing bariatric surgery means making a lifetime commitment to not only changing your anatomy, but also your eating behaviors and lifestyle. Before scheduling an office visit, we ask that you serious consideration to a great deal of information.

The scheduling process takes place in two phases:

At the time of your first call, we will send you

• a procedure pamphlet
• a letter detailing information we will need from you
• a questionnaire
• a letter of acknowledgement

Once you have read the pamphlet, completed the questionnaire, and obtained the necessary information, please send your materials to our office. Once we review your materials, we will call you within a week. This allows time for us to carefully review your information, as we are very thorough in our screening process.
