Hospital Staff Art Requests (password required)



    

Artwork Requests

(UI Hospitals and Clinics Staff ONLY)

To request new artwork for an area that serves patients, or to have artwork moved or removed, the following steps should be taken:

  1. Fill out the Project Art Request Form
  2. Fill out a Green Requisition Form

Please send both forms to Project Art, 8023 JCP. No work will be done by the Project Art staff until both forms are completed and received. Due to the overwhelming number of requests we receive, please allow a minimum of 2-4 weeks for requests to be filled.
