IMPACT (Integrated Multi-agency Program in Assertive Community Treatment)
The IMPACT program supports individuals living in the community, who have a severe or chronic mental illness, by teaching them ways to improve quality of life, self and home care, and resource utilization in an effort to reduce hospitalization.
The program is staffed by an interdisciplinary team who will provide consistent care and assertive client advocacy.
Services provided
- Assistance with symptom monitoring, medication compliance, nutrition, sleep, exercise, money management, shopping, cooking, home maintenance, transportation, and securing or maintaining employment
- Opportunities to participate in social activities and make new friends
- Crisis management

- Assistance with self-care and maximizing functional independence
- Provides opportunities to practice and integrate independent living skills in a home environment to assist in continued recovery process
- May include assistance with grocery shopping, meal preparation, meal outings, time management, goal setting, and social interaction
