Post Hospital Care Planning Unit (PHCPU)
Goal of program: To assist the clinical social workers at the University of Iowa Hospitals and Clinics with discharge planning arrangements for medically complex patients who are in need of placement at a nursing facility, rehabilitation center, or to transfer to another acute hospital.
Services provided:
- Interpretation of medical information
- Level of care assessment
- Identification of community resources
- Communication of medical, psychosocial, and financial information to receiving facilities
- Liaison between UI Hospitals and Clinics and community facilities and agencies
