Iowa Home and Community-Based
Service Waiver Program


The HCBS program helps people in need of additional health care, the kind provided in an institution, while they remain at home. Medicaid covers basic health care services plus services to promote self-care and/or family care. A child, adult, or elderly person may be eligible if s/he meets the program guidelines.

There are five waivers currently available:

  1. Ill and handicapped
  2. AIDS/HIV
  3. Mentally retarded
  4. Elderly
  5. Brain injury

How to apply: If you are interested in applying for the HCBS program, first contact your Department of Human Services. Ask for a Medical Assistance application.

Complete the form and return it to the DHS as soon as possible since your application will be processed based on the date it is received.

You may also need to apply for Supplemental Security Income (SSI) at telephone number 1-800-772-1213. You must be denied SSI before you can qualify for HCBS Waiver program. Therefore, be sure to send your SSI denial statement to the Department of Human Services.

The UI Hospitals and Clinics Child Health Specialty Clinics (319) 353-6172 also have an agreement with DHS to assist the Iowa Model Waiver program.

What to do if you are denied: If you have been denied benefits and wish to appeal the decision, this can be done through either the Iowa Foundation for Medical Care, the Department of Human Services, or the Social Security Administration, depending on the reason for the denial. Contact the DHS or your UI Hospitals and Clinics social worker if you are confused as to where your appeal should be directed.
