• On-call physicians are available for each clinical department
• Faculty consultation is available 8 a.m.-5 p.m., Monday-Friday (no holidays).

To facilitate a referral, please be prepared to provide the following information when you call:

• Patient's name
• Address
• Phone number
• Date of birth
• UI Hospitals and Clinics registration number (if born or previously seen here)
• Type of insurance (i.e. - HMO, Workers' compensation, Medical Assistance)
• Nature of the medical problem
• Services you would like UI Health Care to provide
