Coordinating Patient Admissions

• Trained staff facilitate hospital admissions and patient transfers.
• Calls are triaged based on patient need and emergency transport can be arranged for adult patients requiring air transport and all neonatal patients.
• Information for stabilizing the patient before transport is available.

Information needed to refer a patient:

To facilitate a referral, please be prepared to provide the following information when you call:

• Patient's name
• Address
• Phone number
• Date of birth
• UI Hospitals and Clinics registration number (if born or previously seen here)
• If known, yourType of insurance (i.e. - HMO, Workers' compensation, Medical Assistance)
• Nature of the medical problem
• Services you would like UI Health Care to provide
