• Community health care providers may call anytime to receive an update on a patient's condition
• UI Consult staff members assist UI physicians in distributing patient information to community health care providers.
• You may also call for the results of diagnostic tests and studies

Information needed to refer a patient:

To facilitate a referral, please be prepared to provide the following information when you call:

• Patient's name
• Address
• Phone number
• Date of birth
• UI Hospitals and Clinics registration number (if born or previously seen here)
• Type of insurance (i.e. - HMO, Workers' compensation, Medical Assistance)
• Nature of the medical problem
• Services you would like UI Health Care to provide
