Medical Record Requests

An accurate medical history is the most effective way to get the best possible diagnosis and medical care should you become sick. Before your first appointment, medical records from your most recent primary care provider should be sent to your UI Family Care clinic.

Please fill out an authorization to release medical records form. Once completed, submit the form to your previous provider and they will forward your records to UI Family Care. 

If you are requesting your medical records from UI Family Care, all requests must be received in writing.
