Health Plan Changes for
January 1, 2008 - December 31, 2008


UICare, UISelect, CHIP III will be discontinued as of December 31, 2007.

CHIP II:
The single person's out of pocket maximum is $4,200 and the family out of pocket maximum is $8,400.

Immunizations regardless of age will have $0 co-insurance.

Hearing aid benefits will have a maximum of $2,000 every 5 years.

A treatment plan must be approved by Wellmark following the initial chiropractic visit.

UIChoice:
This is a new plan designed with three levels of coverage, depending on where your health care is rendered. It is designed to give the least out of pocket expense for UI-affiliated providers as well as coverage across the State of Iowa using Blue Choice providers with an out of pocket maximum of $1,700 single/$3,400 per family. Level 3 will support insurance coverage when services are provided by non-level 1 and 2 providers and have a separate out of pocket maximum of $2,000 single/$4,000 per family.

UIGRADCare:
The Graduate student has a choice of using Student Health Service or other UI-affiliated providers for primary care services.

Dependents can be enrolled through age 19 without being a full time student.

SHIP:
No insurance plan changes for 2008.
