The Comprehensive Health Insurance Plan (CHIP II) enables you to obtain coverable health care from almost any provider. Your out-of-pocket costs might be less if you select providers who have contracts with Blue Cross/Blue Shield.
CHIP II enables you to receive major medical care as well as health maintenance care, immunizations, mental health, chemical dependency and prescription drugs.
Mental Health and Chemical Dependency
Benefit coverage is available for services with any of the following providers: psychiatrist, psychologist, licensed social worker, advanced registered nurse practitioner, and licensed marriage and family therapist.
A confidential assessment and up to 4 visits can be obtained at no charge to the patient by calling the UI Integrated Employee Assistance Program at 319-335-2085. Counselors can assist patients with needs, help resolve the problem, or facilitate a referral to a provider based on the assessment.
Prescriptions
All pharmacies in the Iowa City/Coralville/North Liberty area can be used. For plan participants living outside this area, check pharmacy locations. Certain drugs are limited per month, benefit period, or lifetime by drug-specific quantity limitations. The limitations are determined by Wellmark based on Medical necessity. For a list of drugs subjects and quantity limitations, check with your pharmacist, physician or Wellmark.
Important Features of CHIP II plan includes:
- Open access to any providers across the nation. Selecting a Blue Cross/Blue Shield traditional indemnity provider may reduce your out-of-pocket costs.
- Comprehensive coverage for physicals, well-child visits, and other preventive services.
- Access to disease management programs available at the University of Iowa Hospitals and Clinics.
- Single out-of-pocket maximum for both medical and prescriptions of $4,200 per person; $8,400 per family
- Nurse case manager within the UI Health Plans Office to assist you with health care needs.
