PICC tip location:
Per INS standard 42.4
"Central vascular access devices shall have the distal tip dwelling in the lower one third of the superior vena cava to the junction of the superior vena cava and the right atrium."
The junction of the superior vena cava and the right atrium is called the Atrial Caval Junction.
If the x-ray report states:
- The line is in the SVC or the Atrial Caval Junction it is ok to infuse any fluids into this line.
- The line is in the right atrium the line must be pulled back into the SVC or the patient will experience atrial flutter.
- The line is above the SVC (to short) NO TPN, VESICANTS, or properly diluted AI. If it is located short of the SVC it will need to be evaluated on an individual basis to see if it is safe to use in the home.
- If it is located in the jugular vein you can NOT use it for anything. **If a pt. with a PICC line begins to complain of hearing swishing or water when they are flushing they should stop using the line immediately and have and x-ray done to confirm placement. This is usually a sign that the line has migrated into the jugular vein.
