Clinical Coordinator

The Clinical Coordinator at UI Community HomeCare is the first person who will record information for potential home infusion referrals. The coordinator evaluates patients based on information provided and determines whether or not they are appropriate candidates for home infusion therapy. The clinical coordinator will plan the patient's transition from hospital or clinic to home and make the necessary arrangements to ensure that the patient's individual needs are met. The coordinator works closely with the UI Community HomeCare pharmacist, infusion nurses, and inventory clerk to assure services are delivered when and where they are needed.
