When patients begin and continue home infusion therapy, the Home Care Pharmacist provides a plan for their care based on the individual needs and on the therapy that has been prescribed by their physician. The Home Care Pharmacist assures that patients are monitored for response to their infusion therapy and that medications are compounded and shipped or delivered so they always have supplies available. The Home Care Pharmacist works closely with the physician to keep him/her informed of the patient’s response to therapy and progress toward established therapy goals. Our Pharmacist is a certified Home Infusion Pharmacist.
