How to receive birth certificate copies

Birth certificates for all individuals born at University of Iowa Hospitals and Clinics are kept on file at the Johnson County Recorder's Office. Requests for official copies of birth certificates should be addressed to the Recorder's Office at 913 S. Dubuque St., Iowa City, Iowa 52240 or by phone at 319-356-6093.
