- Community health care providers may call anytime to receive an
update on a patient's condition
- UI Consult staff members assist UI physicians in distributing
patient
information to community health care providers.
- You may also call for the results of diagnostic tests and
studies

Information needed to refer a patient:
To facilitate a referral, please be prepared to provide the
following information when you call:
- Patient's name
- Address
- Phone number
- Date of birth
- UI Hospitals and Clinics registration number (if born or
previously seen here)
- Type of insurance (i.e. - HMO, Workers' compensation, Medical
Assistance)
- Nature of the medical problem
- Services you would like UI Health Care to provide
