Coordinating Patient Admissions
- Trained staff facilitate hospital admissions and patient
transfers.
- Calls are triaged based on patient need and emergency
transport can be arranged for adult patients requiring air
transport and all neonatal patients.
- Information for stabilizing the patient before transport is
available.
Information needed to refer a patient:
To facilitate a referral, please be prepared to provide the
following information when you call:
- Patient's name
- Address
- Phone number
- Date of birth
- UI Hospitals and Clinics registration number (if born or
previously seen here)
- Type of insurance (i.e. - HMO, Workers' compensation, Medical
Assistance)
- Nature of the medical problem
- Services you would like UI Health Care to provide
