Coronary Artery Bypass Grafting

On the Day of Discharge

Consults
A nurse will discuss a monitored exercise program so you can continue to strengthen your heart at home. At the University of Iowa Hospitals and Clinics this monitored exercise program is commonly referred to as "CHAMPS".

Tests
Blood sugars will continue to be checked if you have diabetes.

Treatments / Interventions
Same as previous day

Your heart monitor will be removed.

Medications
The Pharmacist will go over your home going medications in detail and give you a list to take home with you. You will be asked if you would like your medications filled locally or here at the hospital.

Diet
The dietician will go over dietary restrictions prior to discharge

Activity
The physical therapist will set up outpatient cardiac rehabilitation

Teaching
Information will be repeated, as needed.

You will be asked:

  • To state name, purpose and dose of medications with aid of written materials.
  • 5 things to report to local doctor if they occur.
  • Why you should not drive any sooner than 4 weeks after surgery.
  • How you plan to modify your activity at home.
  • What emotions you may experience and what to do for them.
  • What changes to make when resuming sexual activity.

Discharge Planning
A visiting nurse will be contacted if you have chosen to have them see you at home.
